·1156· 检验医学 2017 年 12 月第 32 卷第 12 期 中图分类号 : R446.1 Laboratory Medicine,December 2017,Vol. 32,No. 12 DOI : 10.3969/j.issn.1673-8640.2017.12.020 文章编号 : 1673-8640(2017)12-1156-03 文献标志码 : A SYSMEX UF-1000i 尿有形成分分析仪细菌信息在 尿路感染筛查中的应用 高淑芳, 胡 骏, 胡晓波 (上海中医药大学附属龙华医院检验科,上海 200032) 摘要:目的 探究SYSMEX UF-1000i尿有形成分分析仪(简称UF-1000i)细菌信息在尿路感染(UTI)筛查 中的应用价值。方法 收集上海中医药大学附属龙华医院2016年4月27日—9月14日门诊与住院的314例患者清洁 中段尿标本,同时做微生物培养和UF-1000i检测,比较分析UF-1000i细菌信息Rods和Cocci/Mixed与微生物培养结 果。结果 UF-1000i细菌信息Rods预测杆菌的诊断敏感性为76.8%,阳性预测值为70.0%;Cocci/Mixed预测球菌/混 合菌的诊断敏感性为38.1%,阳性预测值为16.3%。结论 UF-1000i提示Rods时,可初步鉴别细菌类型,对UTI早 期筛查、指导临床合理应用抗菌药物、缩短治疗时间有一定价值;而提示Cocci/Mixed时,结果尚不能用于临床。 关键词:尿有形成分分析仪;细菌培养;Rods;Cocci/Mixed Bacteriological information of SYSMEX UF-1000i urinalysis analyzer in screening urinary tract infections GAO Shufang , HU Jun , HU Xiaobo. ( Department of Clinical Laboratory , Longhua Hospital , Shanghai University of Traditional Chinese Medicine,Shanghai 200032,China) Abstract:Objective To evaluate the bacteriological information of SYSMEX UF-1000i urinalysis analyzer in screening urinary tract infections(UTI). Methods A total of 314 specimens of midstream urine were collected from outpatients and inpatients in Longhua Hospital affiliated to Shanghai University of Traditional Chinese Medicine from April 27 to September 14,2016. These specimens of midstream urine were determined by microbiological culturing and UF-1000i. The results of Rods and Cocci/Mixed information of UF-1000i were compared with those of microbiological culturing. Results The sensitivity and positive predictive value of Rods in predicting negative bacillus were 76.8% and 70.0%,respectively. The sensitivity and positive predictive value of Cocci/Mixed in predicting positive coccus or mixed infection were 38.1% and 16.3%,respectively. Conclusions When UF-1000i indicates Rods,it can distinguish the types of bacteria preliminarily,guide the rational use of antibiotics in clinic and shorten the duration of treatment. When UF-1000i indicates Cocci/ Mixed,the results are diverse and are needed to be improved. Key words:Urinalysis analyzer;Bacterial culturing;Rods;Cocci/Mixed 尿路感染(urinary tract infection,UTI) 是 泌尿系统常见的感染性疾病,好发于老年男性及 各年龄段女性,多由细菌感染引起,偶见真菌感 染 [1]。全世界每年约有1.5亿人患UTI [2]。如果未 能得到及时有效的治疗,UTI可能会导致一些严 重的并发症,如肾损伤、脓毒血症等[3],影响患 者的康复。因此,及时、准确地诊断UTI对优化 抗菌药物治疗及改善预后至关重要。 本研究将SYSMEX UF-1000i尿有形成分分 析仪(简称 UF-1000i )的检测结果与 UTI 诊断 “金标准”定量中段尿细菌培养结果进行比较 分析,旨在评估UF-1000i的细菌信息在UTI筛查 中的应用价值,为临床诊断及治疗 UTI 提供快 速、准确的筛查结果。 1 材料和方法 1.1 病例资料 收集
